What Is Perimenopause?

It most often begins in a woman's 40s, can start in the late 30s, and typically lasts several years. Menopause itself is one moment: 12 months in a row without a period.

The rollercoaster matters more than the decline. Progesterone often drops first, which can affect sleep and mood. Estrogen can swing higher than usual before it falls, which can bring heavier periods, breast tenderness and irritability. Those shifts affect your thyroid, stress hormones and blood sugar too, which is why perimenopause can feel like it touches everything.

Meet Chelsie Moore, Your Portland Perimenopause Guide

I'm Chelsie Moore, a functional medicine clinician and integrative dietitian nutritionist, and the founder of Moore Integrative Health in Portland, Oregon. I work primarily with women on hormone health, perimenopause, fatigue, mood and sleep, blood sugar regulation, and autoimmune concerns.

My training includes a master's degree, two years of clinical nutrition residency, a certification in behavior change from the Yale School of Public Health, and Women’s Integrative Health practitioner training with Aviva Romm, MD. My background in behavior change matters here: knowing what to do and actually being able to do it in a full, busy life are two different things. I help with both.

How do I know if I'm in perimenopause?

If you're in your late 30s to 50s and noticing changes in your cycle, sleep, mood, energy or weight, perimenopause is a likely contributor. There's no single test that confirms it. Diagnosis is based mostly on your age, symptoms and cycle changes, and labs help rule out other causes, such as thyroid issues.

What does a functional medicine practitioner do for perimenopause?

A functional medicine practitioner looks for the root causes behind your symptoms, including hormones, blood sugar, stress, thyroid and gut health, and builds a personalized plan. That plan usually includes nutrition, lifestyle, targeted testing and supplements, coordinated with your other providers.

Can perimenopause symptoms be managed naturally?

For many women, yes. Nutrition, blood sugar balance, stress support, sleep strategies and targeted supplements can meaningfully reduce symptoms. Some women also benefit from hormone therapy, and integrative care can work alongside it.

Your OB-GYN is essential for gynecologic care, screenings and prescriptions. My role is complementary. I offer longer appointments, in-depth nutrition and lifestyle support, and a root-cause look at how your whole system is responding to this transition.
